Per minute rate limits by plan

Every plan now has a clear per minute request limit: 15 for Free, 60 for Starter, 150 for Pro.

This replaces the older, looser limit and makes throughput predictable for automated jobs.

Responses include the usual rate limit headers so your integration can back off cleanly.
